Personalised connected care for patients with complex chronic diseases.
Based on the concept of 4P medicine (Predictive, Personalized, Preventive and Participatory), CONNECARE is a solution which monitors patients’ activities and status, thus empowering and providing them with recommendations to self-manage their condition, resulting in substantial improvements in their quality of life.

Our Role
Our main role in this project was to provide technical expertise to the founding team. In addition to that, we were also involved in every step of the way and our main activities were:
- Implementation – software development of the front-end, QA and testing. Integration with the back-end services provided by the client.
- Delivery & support – marketplace submission, deployment, hosting & cloud setup and monitoring.
Our Role

Our Horizon 2020 Project CONNECARE needed to shape and deliver innovative and user-friendly tools for complex chronic patients to better self-manage their condition and feel empowered in their care journey. This was an ambitious challenge and Indigo Verge made a crucial technological contribution to make it happen. They were timely committed and proactive during the whole process and the results were professional, attractive and robust, which is exactly what the end users needed to improve their patient experience and ultimately their quality of life. 🙂
